Observation

Infant classroom that is being used as Toddler classroom today: there is pea gravel all over the floor. 110-300-0165 (2)(a) Toddler classroom that is being used as Infant classroom today: the electrical cord from the AC is on the floor and accessible to the infants. (2)(c) Wooden board have nails exposed. All three playgrounds: The border/frame around the pea gravel have the large screws exposed. School age border/frame: the frame is cracked on one of the corners. 110-300-0165 (3)(a). Pre-school classroom: the carpet in the stairs is torn/ tripping hazard. Toddler playground: walkway is uneven (tripping hazard). 110-300-0165 (3) (e) Outdoor climbing equipment’s (Pre-school and Toddler): Both climbing equipment’s have rust on the steps and other areas of the equipment. 110-300-0165 (3) (g) The light fixtures at the main entrance of the building (area where parent sign in and out) are 110-300-0165 (4) (c)not working. The center does not have hot water. 110-300-0165 (4) (e) School Age classroom: Some of the electrical outlets are not Tamper Resistant. 110-300-0165 (5)(a) Toddler classroom that is being used as an Infant Classroom today: there is an extension cord on the floor accessible to the children. 110-300-0165 (5)(e) Toddler Classroom that is being used as Infant classroom: the AC insert is not connected directly into 110-300-0165 (5)(d)(f).an electrical outlet, it's connected by using an extension cord.
